History
The theater building now known as the Fort Theater was originally built in 1914. When it opened, it was named the Empress, and was considered to rival any moving picture theater in the state at the time. A large stage could show movies on the largest screen in town, but could also accommodate a vaudeville troupe. A six-piece orchestra furnished live music each night. In 1940 a fire gutted the empress, but left the main facade of the building intact. Over the next eight months, a flurry of building activity ensured that the theater would be ready to open by late summer as the new Fort Theatre.

For this reason, our office meets and surpasses all OSHA (Occupational Safety and Health Administration) and CDC (Center for Disease Control) standards. We are confident that you will feel right at home in our office as we welcome all patients as if they were family.

Financial
Our staff will be happy to submit all insurance forms for you and help you to recover the most from your insurance. We will do everything to help you afford the treatment you need and want. For patients who require major work, a complete payment plan can be designed with a payment schedule to meet your financial needs. Or office accepts check, cash, or any major credit card.
Cancellation
If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours notice. We ask for this advance notice so that we can offer this appointment to another patient. A fee may be charged if a patient does not arrive for an appointment without sufficient notice.
